Borderlands 4 continues the franchise’s legacy of chaotic looter-shooter action, setting its latest adventure on the mysterious planet of Kairos. The first major expansion, Mad Ellie and the Vault of the Damned, deepens this experience with a chilling cosmic-horror aesthetic, a new playable Vault Hunter named C4SH the Rogue, and the frigid, derelict-strewn landscapes of The Whispering Glacier. Amidst high-stakes combat and eldritch secrets, the expansion features various side quests that blend the series’ signature dark humor with intense gameplay, most notably the infamous mission known as Doc of the Bay. Unlocked after the story quest "What Lies Dreaming," Doc of the Bay takes players deep into the rusted bowels of the derelict ship, DCN Feuermann. Upon entering the medical bay, the player encounters a panicked, parasite-ridden DAHL soldier who delusionally insists the Vault Hunter is a qualified physician. This triggers a frantic, time-sensitive mini-game where the player must use firearms to shoot glowing holographic parasites while navigating a failing medical console. The term "Doc of the Bay failed" refers to the grim conclusion that occurs if the player fails to clear the parasites within the one-minute timer. As time expires, the soldier meets a gruesome end, and the Vault Hunter responds with typical cold indifference, citing their earlier warnings that they weren't a real doctor. In a twist of dark satire, the game rewards the player with loot and experience regardless of the patient's death. While intended as a comedic narrative outcome, this "failure" became the default experience for most players due to persistent technical bugs. Reports of inconsistent hit detection, unclear holographic angles, and un-targetable parasites rendered the surgery nearly impossible for many. Because the mission cannot be replayed, players were forced to accept the soldier’s death as canon to their playthrough. Ultimately, the bug-ridden nature of the quest inadvertently created a quintessential Borderlands moment: a darkly funny, absurd, and entirely chaotic catastrophe that perfectly mirrors the unpredictable and often incompetent nature of life as a mercenary on Kairos.
